           PFC offers a generous benefits 
            package, including medical, prescription drug, dental, life insurance, 
            long term disability insurance, and a 401(k) plan.  
            
          Medical:
            PFC pays a major portion of the cost of employee coverage, and you 
            contribute the rest through pre-tax payroll deductions. The plan includes 
            coverage for:
            · Hospital
            · Surgery
            · Maternity
            · Well Baby Care
            · Routine adult physicals
            · Routine pediatric care (to age 19)
            · Routine injections and immunizations
            · Outpatient lab, x-rays, and testing
            · Eye exams
            · Mental health and substance abuse treatments
            · Prescription drugs
          Dental:
            As with the medical plan, PFC pays a major portion of the cost of 
            employee dental coverage, and you contribute the rest through pre-tax 
            payroll deductions. The dental plan includes coverage (at varying 
            percentages of allowable cost) for:
            · Preventative and Diagnostic- Exams, cleanings, x-rays, fluoride 
            treatment and fillings.
            · Basic restorative care-extractions, root canals 
            · Major restorative care- crowns, bridgework, dentures
            · Orthodontia for dependents to age 19.
          Life Insurance/Disability Insurance:
            Life insurance is provided to all employees at no cost, and long-term 
            income replacement is offered in case an illness or injury prevents 
            you from working for longer than 6 months.
          401(k) Plan:
            PFC, through its parent corporation Parlex, is proud to offer an excellent 
            401(k) plan to help you save for your future. You may defer up to 
            15% of your pay on a pre-tax basis. You are always fully vested for 
            your own contributions.
