Here is a simple mortgage documentation checklist to help you in gathering the information you'll need to apply for a mortgage. Gathering this information also helps us expedite your application to give you the quick turnaround you need.

All Mortgage Loans

Information Needed At Time of Application:
1. Two most recent year-to-date pay stubs.
2. Two most recent W-2 Forms.
3. Self-employed Only: Two year personal and corporate tax returns and year to date financial statement.
4. Documentation of any other income you choose to disclose (pension, child support, rental income, investment income).
5. Three months statements for all checking, savings, retirement, and investment accounts (including 401K, money market, pension).
6. Account name, number, monthly payment and approximate balance for all outstanding credit debts (auto, personal, credit cards, and student loans).
7. Twelve months canceled rent checks or landlord name and address (if applicable).
8. Fully executed purchase and sales agreement for lot, or copy of deed if owned.


Construction to Permanent Loans Only

Information needed at or immediately after application:
1. Copy of builder's Massachusetts license.
2. Copy of signed building contract with a General Contractor.
3. Plans and specifications, inclusive of elevations and dimensional floor plan.

Information needed 72 hours prior to a scheduled loan closing:
1. Copy of septic disposal works permit, when applicable.
2. Copy of building permit when available.
3. Signed Construction Disbursement Schedule.
4. Copy of Well Water Certificate when applicable.
